The Campbell School District is a very proud and culturally diverse community that takes a sincere interest in the education of its children and families.
  • Memorial Student Handbook:
    A compilation of policy and procedures of Memorial High School and Campbell City School District.  This document includes the Student Code of Conduct, Disciplinary Procedures, Grading Procedures, along with other important information that will assist parents during their child's tenure at Memorial High School.
 
  • Middle School Student Handbook:
    A compilation of policy and procedures of Middle School and Campbell City School District.  This document includes the Student Code of Conduct, Disciplinary Procedures, Grading Procedures, along with other important information that will assist parents during their child's tenure at Middle School.

  • Waiver Form:
    This form is a waiver, that relieves Campbell Memorial High School for any financial responsibility for injuries sustained to the student athlete during participation in any sport, practice, fitness, or game participation.
 
  • Emergency Medical Form:
    To enable parents and guardians to authorize the provision of emergency treatment for children who become ill or injured while under school authority, when parents or guardians cannot be reached.
 
  • Physical Exam Form
    A section completed by the student and his/her parents(s) or legal guardian(s) before participation in order to help detect possible risks. This is also another section that is to be completed by physician or staff after history and consent forms are completed.
 
  • Lunch Form:
    Children need healthy meals to learn. CMHS offers healthy meals every school day. Breakfast costs $1.00 lunch cost $2.00. Your children may qualify for free meals or for reduced price meals.  Reduced price is $.30 for breakfast and $.40 for lunch
 
  • Work Permit:
    Application for minor work permit and Physician's Certificate for minor work permit.  All three parts of the application must be filled out and signed.  The information can then be put in the computer.  The work permit is computer generated thru the State of Ohio's website.  When this is completed, it will be printed at Memorial High School and ready for the student to sign.  THIS PROCESS TAKES AT LEAST 2 DAYS TO COMPLETE!
