All new student enrollment requires an appointment in our Enrollment Office. Parents or legal guardians fill out a completed enrollment packet along with all required documents to complete the registration process. The custodial parent or guardian MUST be a resident of the Campbell City School District. If entering kindergarten the child must reach age 5 on or before September 30, 2018.

The following documents are REQUIRED for registration.
Please have everything when you come to your appointment.
____Current Immunization Record (A physician’s verification of immunizations.)
____If Applicable Proof of Custody – most recent court order/ foster placed paperwork (A certified copy of a judgment entry, court order, or decree signed by a judge and filed with the Clerk of Courts must be presented)(NOT a notarized paper)
____Parent/Guardian’s Driver's License or State ID
____Three (3) Different Current Proofs of Residence
PROOF OF RESIDENCY
All documents must be in the name of the residential/custodial parent or legal guardian with a service address that is the same as their residential address and include the date, which must be within 30 days.
Please present one document from the following:
IF YOU ARE A HOMEOWNER:
- Mortgage statement
- Most recent property tax bill
- Homeowners insurance statement
- Purchase agreementwith mortgage statement to be submitted after closing on sale of home
IF YOU ARE A RENTER:
- Current lease agreement that includes the address of your residence and the signature page
- Renter's insurance statement
In addition, please provide two of thefollowing:
- Government document showing the parent/guardian's address, including but Not limited to: benefits received from Job &Family Services, Social Security Administration, or Bureau of Workers’ Compensation
- Utility bill: Gas, Electric, Water, Sewer or Internet
- Driver's license (with a current address)
- Credit card statement
- Car insurance statement
- Car/Student loan statement
The following items will NOT be accepted:
- Non-compliant driver's license
- Cell phone bill
- Tax forms
- Solicited mail
* If you are a resident of a YMHA property, you need a copy of the lease from the YMHA office with you and your children’s names on the lease.

- Kirwin Homes All require a lease to reside. If you are staying with someone who lives at one of the above mentioned addresses you must provide a letter from the leasing office stating they are allowing you to live there and for how long.

NOTICE OF LEGAL OBLIGATION Parents/Guardian of all students are required by the Ohio Revised Code (ORC) to inform school officials of any of the changes listed below.
1. Change of Address: You must, within 10 calendar days of your move, bring proof of your new residence to the building secretary.
2. Change of Phone Numbers: Notify the secretary of the school your child attends.
3. Change of Legal Custody or Guardianship: Any and all current court orders from Domestic Relations, Juvenile, Probate or any other court which has exercised jurisdiction over the custody, guardianship or residence of the children as per Ohio Revised Code 3313.672 must be included with this package.
4. All Temporary Restraining Orders and Protection Orders involving school premises.
5. Student expulsion or exclusion from any school pursuant to Ohio Revised Code 3301.121 and 3313.662.
