Welcome to the Campbell City School District! Home of the Red Devils!

All new student enrollment requires an appointment in our Enrollment Office. Parents or legal guardians fill out a completed enrollment packet along with all required documents to complete the registration process.  The custodial parent or guardian MUST be a resident of the Campbell City School District.  If entering kindergarten the child must reach age 5 on or before September 30, 2018.

  • The following documents are REQUIRED for registration, please have  everything when you come to your appointment.

    • ____Birth Certificate (School officials may request documentation to show that a student falls within the school district’s minimum and maximum age requirements. School districts typically accept a variety of documents for this purpose, such as a religious, hospital, or physician’s certificate showing date of birth; an entry in a family bible; an adoption record; an affidavit from a parent; a birth certificate; or previously verified school records.)

    • ____Social Security Card

    • ____Current Immunization Record (A physician’s verification of immunizations.) 

    • ____If Applicable Proof of Custody – most recent court order/ foster placed paperwork ( A certified copy of a judgment entry, court order, or decree signed by a judge and filed with the Clerk of Courts must be presented)(NOT a notarized paper)

    • ____Parent/Guardian’s Driver's License or State ID

    • ____Three (3) Different Current Proofs of Residence
       

       * If you are a resident of a YMHA property you need a copy of the lease from the YMHA office with you and your children’s names on the lease.

      All require a lease to reside.  If you are staying with someone who lives at one of the above mentioned addresses you must provide a letter from the leasing office stating they are allowing you to live there and for how long. 


      In order to establish that you are a current resident of Campbell and to comply with this state law, you as the parent/guardian must provide (3) valid and recent forms of documentation from any of the following 14 criteria. Recent documentation means within the last 30 days. Each document must include the current address of the guardian/parent’s and students primary residence. 

    • Most recent City of Campbell tax return  if issued within the last 30 days.

    • One current utility bill issued within the last 30 days: Example: electric, telephone, cable/satellite, water, gas.

    • Documents MUST show parent/guardian’s full name, address and date
      (date must be within 30 days of the date you enrolling with.)

    • A recent paycheck(s)/stub showing employer and place of residence issued within the last 30 days.

    • Public Assistance Check Stub – with your address issued by a government public assistance agency within the last 30 days.

    • Current bank statement or Major Credit Card bill showing name and address of parent/guardian issued within the last 30 days.

    • Mortgage Account - or proof of home ownership, certificate of occupancy for two family or single home rental, property tax bill or receipt of payment, deed, landlord/tenant agreement within the last 30 days.

    • Installment Loan Contract - from a bank or other financial institution within the last 30 days.

    • Recent Ohio certificate of auto title or auto mail-in Renewal Notice issued within the last 30 days.

    • State of Ohio Driver’s License issued within the last 30 days.

    • Professional License - issued by an Ohio government agency within the last 30 days.

    • Concealed Carry Weapons Permit issued within the last 30 days.

    • Social Security Administration Document issued within the last 30 days.

    • Jobs and Family Services administration document issued within the last 30 days

    • Bill issued within the last 30 days: Example: medical, dental, car insurance or cell phone

    • Child Support documents/payment statement/Stub within the last 30 days

  • ____Last report card / Transcripts / withdraw grades
     

     

    NOTICE OF LEGAL OBLIGATION Parents/Guardian of all students are required by the Ohio Revised Code (ORC) to inform school officials of any of the changes listed below.

  • 1. Change of Address: You must, within 10 calendar days of your move, bring proof of your new residence to the building secretary.           

  • 2. Change of Phone Numbers: Notify the secretary of the school your child attends. 

  • 3. Change of Legal Custody or Guardianship: Any and all current court orders from Domestic Relations, Juvenile, Probate or any other court which has exercised    jurisdiction over the custody, guardianship or residence of the children as per Ohio Revised Code 3313.672 must be included with this package. 

  • 4. All Temporary Restraining Orders and Protection Orders involving school premises. 

  • 5. Student expulsion or exclusion from any school pursuant to Ohio Revised Code 3301.121 and 3313.662.
