Child must be 5 years old by Sept. 30, 2025 & live in Campbell.

Pick up a registration packet starting March 10

Bring the following to your appointment:

Birth Certificate

Current Immunization Record (Fax: 330-799-6373)

Parent/Guardian’s Driver’s License or State ID

3 different proofs of residency (within the last 30 days)

If applicable, custody paperwork (must be court-filed, not notarized)
